Also found it in my junk folder, but I got it at least.

In the mission Flambeaux gets rescued from Freakshow, and SAYS she's going to come along to help "burn the place down," but she never moves from where you rescued her, and the spawns are set for a team of 2 throughout the rest of the mission.
